Introduction
Scrambler Therapy Certified™ is the foundational certification program for providers who want to integrate Scrambler Therapy® into clinical practice. Conducted on-site at your location or optionally at one of our training facilities, the training combines didactic instruction, live patient treatment, and supervised hands-on application to ensure physicians and clinical staff understand how to implement the 4-5-4 Methodology™ in real world treatment workflow.
This certification is intended for the supervising physician and any trained team members who will participate in treatment delivery. Because clinical outcomes depend on correct application, everyone performing Scrambler Therapy® as part of the treatment process must be certified.
The Key to Clinical Success in Scrambler Therapy®
The 4-5-4 Methodology™ is the structured clinical framework used to guide electrode placement, signal delivery, and treatment optimization in Scrambler Therapy®. It is the official, stand-alone methodology developed and approved by the inventor of Scrambler Therapy®, Giuseppe Marineo.
This methodology provides a repeatable framework for systematically iterating electrode placement and stimulation parameters to achieve optimal patient response during each treatment session. Rather than relying on static placement, providers are trained to adjust in real time based on patient feedback until effective pain relief is achieved.

Preparation
Preparation begins before the training team arrives. In the weeks leading up to certification, we work with your clinic to build an initial patient schedule, identify appropriate cases, and prepare your team for a productive three-day hands-on experience.
Training is typically conducted over three full days and is designed around live treatment of real patients within your clinic. Most trainings include approximately 6 to 8 patients per day, allowing providers to see a range of pain patterns, treatment responses, and electrode placement needs across multiple cases.
Physicians are encouraged to include challenging chronic neuropathic pain patients as part of the training experience. This helps providers build confidence with the nuances of case evaluation, placement strategy, and treatment progression. Training is commonly scheduled Monday through Wednesday so patients can continue treatment later in the week if needed, though alternate scheduling can be arranged to fit your practice.

Day 3 is focused on independent execution and clinical confidence.
Providers lead treatments with minimal guidance
Full responsibility for electrode placement and treatment flow
Real-time oversight and correction from the training team
Reinforcement of the 4-5-4 Methodology™ and clinical consistency
Final review and competency validation
The training concludes with certification, formally recognizing the provider’s ability to safely and effectively deliver Scrambler Therapy®.
By the end of the three-day experience, providers are not just familiar with the device; they are prepared to integrate Scrambler Therapy® into their practice with confidence from day one.

Attendance
The Scrambler Therapy Certified™ program is designed for the supervising physician and the clinical team members who will support or perform treatment within the practice. This may include physicians, nurse practitioners, physician assistants, registered nurses, and other medical professionals, depending on clinic workflow and applicable supervision requirements.
Because Scrambler Therapy® outcomes depend on proper application of the treatment methodology, any team member involved in treatment delivery should complete certification.
Post-Training
At the completion of the three-day certification, your team should be prepared to begin treating patients within your clinic using the principles, workflow, and 4-5-4 Methodology™ covered during training. The goal of certification is not simply device familiarity, but true clinical readiness, allowing your practice to implement Scrambler Therapy® with confidence.
Following training, our team remains available for continued clinical support. We provide follow-up in the weeks after certification to ensure a smooth transition into practice and to assist with any challenging cases or unexpected patient responses. Providers are encouraged to leverage this support as they continue building experience and refining their approach.
As your team gains experience and begins encountering more complex cases, progression to Scrambler Therapy ADVANCED Certified™ is recommended to further develop expertise in electrode placement, treatment strategy, and clinical decision-making.
